Dr. Thomas Kempen is an Assistant Professor at Utrecht University in the Faculty of Science, Department of Pharmacoepidemiology and Clinical Pharmacology. He also holds a Visiting Researcher position at Uppsala University in Sweden.
His research focuses on:
- Pharmacist roles in primary care and transitions of care
- Medication review processes and pharmacist prescribing
- Polypharmacy management in older patients
- Interprofessional education and clinical decision-making
- Development of medication safety tools
Recent work includes the IMPACT-care study on medication communication during care transitions and evaluations of pharmacist prescribing in Dutch primary care. He contributes to journals like the International Journal of Clinical Pharmacy and Journal of Interprofessional Care, and participates in EU-funded projects on polypharmacy management.
