Blair Smith is a Clinical Professor (Teaching and Research) of Population Health Science at the University of Dundee, based in the School of Medicine's Division of Population Health and Genomics. He holds the Chair in Population Health Science and serves as Clinical Co-director of the Tayside Clinical Trials Unit. Previously, he was a Consultant in Pain Medicine at NHS Tayside and Professor of Primary Care Medicine at the University of Aberdeen (until 2011). His roles include Specialty Adviser to the Scottish Government’s Chief Medical Officer and former National Lead Clinician for Chronic Pain (2014–2021). He co-chairs the International Association for the Study of Pain’s Global Alliance of Pain Patient Advocates (GAPPA) and has led initiatives like the Scottish Pain Research Community (SPaRC).

- Genetic studies (e.g., Generation Scotland with 30,000 participants)
- Primary care management strategies and self-management approaches
- Biological mechanisms linking adverse childhood experiences to chronic pain and multimorbidity
- Development of algorithms for chronic pain identification in electronic health records

Awarded the British Pain Society Medal (2018), Clarivate Highly Cited Researcher (2020–2022), and Royal Society of Edinburgh Fellowship (2023). Recognized as a World Expert in Chronic Pain by Expertscape and ranked #2 globally by ScholarGPS (2024).
