Thomas J. Montine, MD, PhD is Professor of Pathology and Chair of the Department of Pathology at Stanford University School of Medicine. He holds the Stanford Medicine Endowed Professorship in Pathology and leads the Montine Laboratory, which focuses on the structural and molecular bases of cognitive impairment in aging. Previously, Dr. Montine served as Chair of the Department of Pathology at the University of Washington and Director of the University of Washington Alzheimer's Disease Research Center, one of the original 10 Centers in the US.
Dr. Montine's educational background includes a BA in Chemistry from Columbia University, a PhD in Pharmacology from the University of Rochester, and an MD and CM from McGill University. His postgraduate medical training was completed at Duke University, where he specialized in pathology and completed residencies in Anatomic Pathology and Neuropathology. He is board certified in both Anatomic Pathology and Neuropathology by the American Board of Pathology.
Dr. Montine's research primarily focuses on Alzheimer's disease, dementia, and neuropathology, with particular emphasis on understanding the relationship between neuropathological changes and cognitive function in the oldest-old. His recent work has made significant contributions to understanding cognitive resilience to Alzheimer's disease - the phenomenon where some individuals maintain cognitive function despite having significant Alzheimer's pathology. His laboratory employs a multidisciplinary approach combining neuropathological examination with proteomic, genomic, and clinical data analysis.
His work has revealed that dementia in the oldest-old is rarely due to a single pathology but rather results from multiple concurrent neuropathological changes including Alzheimer's pathology, TDP-43 encephalopathy, vascular injury, hippocampal sclerosis, and Lewy body disease. His research with The 90+ Study indicates that approximately 59% of centenarians and 47% of nonagenarians have at least four neuropathological changes, highlighting the complexity of cognitive impairment in advanced age.
Dr. Montine has received significant recognition for his work, with a D-Index of 139 and over 115,000 citations. His research has been published in numerous high-impact journals including Nature Medicine, The Lancet Neurology, and Nature Neuroscience. He has been instrumental in developing diagnostic criteria and research frameworks for neurodegenerative diseases, including contributions to the NIA-AA Research Framework for Alzheimer's disease.
As Chair of the Department of Pathology at Stanford, Dr. Montine oversees a major academic pathology department and continues to lead innovative research in neuropathology and cognitive aging. His laboratory maintains active collaborations with researchers across multiple institutions, contributing to large-scale studies of aging and dementia.
