Paul Eleazer

G. Paul Eleazer, MD, FACP is a Clinical Professor in the Department of Internal Medicine at the University of South Carolina School of Medicine, with prior roles as Associate Chair for Veterans Affairs at the University of Utah School of Medicine (until 2019). He currently serves as Acting Associate Director for Clinical Innovations at the Salt Lake City VA Medical Center GRECC (Geriatric Research, Education, and Clinical Center).

• Specialist in Geriatrics and Internal Medicine, triple board-certified by the American Board of Internal Medicine (including geriatric medicine subspecialty)
• Founded the Geriatrics Division and Geriatrics Fellowship Program in South Carolina
• Principal Investigator for NIH-funded PREVENTABLE trial (statins and dementia prevention) and GHELP-TC (care transitions for veterans)

His research spans geriatric clinical innovations, aging education programs, and elderly healthcare policy. Publications focus on care transitions, geriatric curriculum integration, and aging perception studies. Scientific honors include Fellow of the American College of Physicians.

Dr. Eleazer has mentored medical students and residents through initiatives like the Senior Mentor Program and Aquifer Geriatrics online curriculum. His work addresses Veterans' healthcare and age-friendly systems, with emphasis on transitions of care and chronic care management for older adults.
