James C. Brust

James C. Brust is a Professor of Medicine with joint appointments in the Divisions of General Internal Medicine and Infectious Diseases at Montefiore. He maintains an active clinical practice as an infectious diseases subspecialist at the Montefiore Center for Positive Living/Infectious Diseases Clinic, serving English- and Spanish-speaking patients.

His research focuses on the dual epidemics of multidrug-resistant tuberculosis (MDR TB) and HIV in South Africa, examining treatment outcomes of co-infected patients, transmission patterns of drug-resistant TB, pharmacology of repurposed TB medications, and genetic mechanisms of drug resistance. Key projects include NIAID-funded research on bedaquiline resistance emergence and leadership of a randomized clinical trial investigating therapeutic drug monitoring for linezolid in rifampin-resistant TB patients.

Dr. Brust serves on multiple state and national guidelines committees for HIV and tuberculosis management. His clinical expertise spans Internal Medicine and Infectious Disease subspecialty care.
