Rachel B. Chalmer, M.D., is an attending physician at Montefiore Einstein and an Associate Professor of Geriatrics at Albert Einstein College of Medicine. Her clinical focus includes consultative care for individuals with cognitive impairment or dementia and their families. She is board certified in Internal Medicine and Geriatrics by the American Board of Internal Medicine and holds membership in the Gold Humanism Honor Society, American College of Physicians, and American Geriatrics Society.
Dr. Chalmer earned her B.A./B.S. in Spanish, Public Health, Pre-Medical Studies, and Medical Humanities from the University of South Carolina (2004), followed by an M.D. from the Medical University of South Carolina (2008). She completed an internship and residency in Primary Care and Social Internal Medicine at Montefiore Einstein (2011) and a Geriatric Medicine Fellowship at Montefiore Medical Center (2017).
Her research emphasizes early detection of cognitive disorders, reducing health disparities in geriatric care, and improving dementia care practices. She has led and co-investigated multiple funded projects, including an NIH grant (UG3NS105565) for a clinical trial on dementia detection. Her work has been disseminated through peer-reviewed journals and invited presentations.
Dr. Chalmer has received notable awards for her teaching and clinical contributions, such as the Leo M. Davidoff Society Award and the Geriatrics Mentor Award. She currently serves as Lead Study Physician for Dr. Joe Verghese’s NIH-funded trial and is faculty in Montefiore’s Center for Excellence in Alzheimer’s Disease (CEAD).
Her educational roles include mentoring residents and fellows in Geriatrics, teaching medical students through the Geriatrics Clerkship, and facilitating small-group clinical discussions. She has been recognized for teaching excellence with the 2019 Montefiore Einstein Medical Student Teaching Star Program Commendation Letter.
