Rachel Forman, MD is an Assistant Professor of Neurology at Yale School of Medicine and a stroke neurologist at Yale New Haven Hospital. She treats patients with stroke-related conditions both in the hospital setting and at the Guilford stroke clinic. Dr. Forman serves as the faculty advisor for the Yale Stamp Out Stroke program and is the Yale site Principal Investigator for the REDUCE trial, which evaluates blood pressure treatment among patients with intracerebral hemorrhage.
Dr. Forman completed her medical degree at Chicago Medical School in 2015, followed by a neurology residency at Rush University Medical Center (2019) and a vascular neurology fellowship at Massachusetts General Hospital (2020).
Dr. Forman's research focuses on critical aspects of stroke care and prevention, with particular emphasis on hemorrhagic stroke, ischemic stroke, neuroimaging, and stroke rehabilitation. She is deeply committed to addressing racial and ethnic disparities in stroke care and has developed community-based educational initiatives to improve stroke awareness and outcomes in underserved populations. Her clinical work centers on stroke prevention and post-stroke blood pressure control, recognizing these as critical factors in improving long-term outcomes.
Dr. Forman's publications demonstrate a consistent focus on practical clinical questions in stroke diagnosis and management, with emphasis on improving systems of care and understanding stroke etiologies. Her work spans both ischemic and hemorrhagic stroke, with particular attention to subarachnoid hemorrhage and emergency department stroke protocols, reflecting her commitment to translating research findings into improved clinical practice.

As faculty advisor for the Yale Stamp Out Stroke program, Dr. Forman mentors medical students and residents in community outreach initiatives. She is actively involved in several clinical trials including the REDUCE trial (as Yale site PI), CAPTIVA, and ASPIRE, which investigate critical questions in stroke prevention and treatment. Her recent grant awards will support the expansion of the Stamp Out Stroke program to better address the unique needs of under-resourced communities in New Haven, with a focus on developing culturally appropriate educational materials that address specific local barriers to stroke care.
Dr. Forman leads the Stamp Out Stroke initiative, which partners with community organizations to develop situation-based educational content. She works closely with the Community Action Agency of New Haven and health technology company MedExplain to create relatable stroke education materials that address specific barriers to calling 911 and accessing healthcare in different neighborhoods across New Haven.
