Jelena Lewis

Dr. Jelena Lewis is an Associate Professor of Pharmacy Practice at Chapman University School of Pharmacy (CUSP) where she also serves as the Director of Residency and Fellowship Programs. She is an Advanced Practice Pharmacist and Board-Certified Ambulatory Care Pharmacist with clinical experience at Providence Medical Foundation in Orange County, CA, where she developed and implemented a chronic disease management program.

Dr. Lewis received her bachelor's degree in biochemistry with a minor in mathematics from the University of California Los Angeles and her Doctor of Pharmacy degree from the University of Southern California, where she was a member of Rho Chi Academic Honor Society. She completed her PGY-1 pharmacy practice residency with an emphasis in ambulatory care and community pharmacy practice at USC, during which she served as an Adjunct Assistant Professor of Clinical Pharmacy. Prior to her academic career, she worked as a clinical pharmacist at Cedars-Sinai Medical Care Foundation managing patients with chronic conditions including diabetes, asthma, dyslipidemia, and hypertension.

Her primary research interests focus on pharmacist-run chronic disease management program implementation and outcomes within Accountable Care Organizations and Patient-Centered Medical Homes, as well as pharmacy student leadership development. Analysis of her recent publications reveals consistent work in diabetes management, cardiovascular outcomes, immunizations, and innovative pharmacy practice models. Her scholarly output demonstrates expertise in translating clinical evidence into practical pharmacy applications across multiple therapeutic areas.

Dr. Lewis serves as a preceptor for pharmacy students and residents during clinical rotations. Her leadership extends to her role as Director of Residency and Fellowship Programs at CUSP and her past service as APhA-APPM Medical Home/ACO SIG coordinator and chair of the Education and Resources Committee. Her work in developing chronic disease management programs demonstrates practical application of her research interests in accountable care models. She has contributed to multiple collaborative research projects focused on diabetes management, cardiovascular outcomes, and immunization practices.
