Brian Hilgeman

Brian Hilgeman, MD, AAHIVS, FACP is an Associate Professor at the Medical College of Wisconsin (MCW) with a dual role in clinical care and academia. He specializes in internal medicine, focusing on vulnerable populations including HIV patients, LGBTQ communities, and individuals with substance use disorders. His clinical practice is split between Froedtert Hospital Specialty Clinics and the Sixteenth Street Community Health Centers, where he also serves as Medical Director of Research, Education, Quality Improvement, and HIV.

Dr. Hilgeman holds a Doctor of Medicine from the University of Wisconsin-Madison School of Medicine and completed his residency at Hennepin County Medical Center. His research emphasizes improving empathy in medical education through reflective practices, optimizing systems for complex patients, and advancing community health initiatives.

Key research themes include narrative reflections for professional development, quality improvement in Federally Qualified Health Centers, and telehealth strategies for high-risk patients. His work has contributed to interventions targeting health equity in vaccination outreach and patient-centered anal cancer screening.

Dr. Hilgeman is a Fellow of the American College of Physicians and holds HIV Specialist certification (AAHIVS). His clinical expertise spans chronic disease management, LGBTQ care, and gender-affirming hormone therapy. He is deeply involved in training future clinicians through resident continuity clinics and medical student mentorship.
